Vendor packets usually fail because one half is missing.
A basic vendor packet often means W-9 plus COI. The W-9 supports tax setup. The COI supports proof of coverage. The fastest review is to check both together and identify what is missing before sending it back.

If someone asked for a vendor packet, gather the W-9, the COI / ACORD 25, and the request details showing certificate holder, insured requirements, and any endorsement language.
What to gather
- Completed W-9.
- COI / ACORD 25 certificate.
- Certificate holder name and address.
- Additional insured, waiver of subrogation, workers comp, umbrella, or auto requirements if requested.
- Requester or client contact email.
